Request Photo Scheduling

Click Here to create PDF Form to request scheduling of your portrait for the Church Directory.
That will open a form that you can fill in the blanks and submit after you get to the bottom of the form. Once you do that a PDF file will be emailed to you and you cam print that out and take it to the Church Office and they will hold it for Paul Mayer to pick up and he will call you to schedule your appointment time.

You can see an Example portrait below. All portraits will be retouched and a copy of your portrait image file will be emailed to you to have prints made as you desire.
